We’ll make the transition to our office easy and hassle-free. Before your first appointment, we invite you to fill out a medical history form. This information will help us get to know you, and will help our practitioners develop treatment plans tailored to your specific needs or concerns.

  • Appointments
  • Please bring your insurance information to your first appointment, as well as any other relevant financial documents (including student cards)
  • Arriving to your appointment 10 minutes early will ensure our staff have sufficient time to process your documents and complete your treatment
  • Our administrators offer appointment reminders via email or phone for your convenience, and you are able to opt-out of this service at any time!
  • Cancellation Policy
  • As a courtesy to our patients and hard-working staff, our office requires 48h notice for cancellations or changes to your appointment. Last minute cancellations impact our dentists’ schedule and our patients’ time and comfort; we thank you for your understanding!
  • If you are feeling unwell, or are experiencing flu-like symptoms, please inform our administrators via phone. For the health and safety of our dental providers, patients, and yourself, you may be advised to stay home to recover; we will gladly reschedule your appointment!
